It's a solid general template, but consent requirements vary by treatment and jurisdiction — review it against your own regulatory and insurance requirements before use.

Should consent be a separate form from intake?

Yes — intake is about the client's history and happens once; consent is per-treatment and should be signed for each procedure (or course of treatments) you perform.

Can I make some consent forms private to practitioners?

On paper, that's a filing-cabinet problem. In Wemu, forms can be marked private so only the practitioners you choose can open responses — front desk can send them but never read them.
